Integrinα5β1在卵巢上皮性肿瘤的表达及临床意义

　　摘 要 目的：通过研究integrinα5β1(整合素α5β1)在卵巢上皮性肿瘤中的表达，探讨其与卵巢癌淋巴结转移的关系。方法：用免疫组化ElivisionTM法检测80例卵巢上皮性肿瘤中integrinα5β1的表达;运用图像分析系统进行定量检测。结果：①定性及半定量结果：integrinα5β1的表达按良性、交界性及恶性的顺序呈递减趋势，组间均有显著差异(P<0.01);integrinα5β1淋巴结转移组与未转移组间表达有显著差异(P<0.01)。②定量检测结果：integrinα5β1表达的阳性细胞面积按良性、交界性、恶性的顺序呈递减趋势，灰度呈递增趋势。结论：integrinα5β1的表达与肿瘤的侵袭、转移有关;integrinα5β1表达下降或缺失，提示肿瘤进展，恶性度高，预后不良;计算机图像分析系统为卵巢上皮性肿瘤的自动化诊断、淋巴结转移及预后评估有重要意义。

　　The expression of integrinα5β1 and its Clinical Significance in Ovarian Epithelial Neoplasms

　　Abstract Objective：Through the investigating the molecular expressions of integrinα5β1　in ovarian epithelial neoplasms, this study is trying to explore the relationship between the lymphatic spread of the tumor with this molecule. Method: 80 cases were examined through ElivisionTM, an immune histochemistry method, for the expression of integrinα5β1 in ovarian epithelial neoplasms. The system of image analysis was used to measure the expression of this molecule quantitatively. Results:①Qualitative and semi-quantitative results: The expression levels of integrinα5β1 in the ovarian epithelial neoplasms have downward tendency in the order of benign, boundary and malignant neoplasms and there is significant difference on the expression levels of integrinα5β1 (P<0.01). There is significant difference on the expression levels of integrinα5β1 between the group with metastases and the group without metastases(P<0.01).②Quantitative examination result: The total positive cell area of the integrinα5β1 expression in the tumor has downward tendency in the order of benign, boundary and malignant cases, and the gray degree has upward tendency. Conclusions: The molecular expressions of integrinα5β1 is related to the tumor invasion and its metastases. Decreasing or lack of the expression of integrinα5β1 suggest the rapid development, high degree of malignancy and poor prognosis; The study also shows the improtance of using the computer image analysis for automatic diagnosis of the ovarian epithelial neoplasms，lymphatic metastases and its prognostic estimation.

　　卵巢肿瘤是女性生殖器常见肿瘤,至今缺少有效的早期诊断方法。近来研究显示卵巢癌细胞上存在黏附因子可促进或调节卵巢癌细胞与腹膜间皮细胞的相互作用，在卵巢癌的浸润转移中起重要作用[1]。本试验主要研究黏附因子integrinα5β1在卵巢上皮性肿瘤的表达及其意义。

　　1 材料与方法

　　1.1 材料来源

　　收集我院病理科1998年～2003年卵巢存档蜡块，为手术切除及根治标本，其中恶性卵巢上皮性肿瘤54例(浆液性43例，黏液性11例)，年龄33岁～70岁，平均52.39岁，术前均未行化疗及放疗;交界性卵巢上皮性肿瘤16例(浆液性13 例，黏液性3例)，年龄22岁～59岁，平均37.44岁;良性卵巢上皮性肿瘤10例(浆液性8 例，黏液性2例)，年龄32岁～70岁，平均41.9岁。

　　1.2 方法

　　4μm 切片，常规HE染色和免疫组化ElivisionTM法染色。所用integrinα5β1抗体(小鼠抗人单克隆抗体，浓度1∶150)为Chemicon公司产品，第二代免疫组织化学ElivisionTM+试剂盒、DAB显色剂均购自福建迈新公司。用已知表达阳性卵巢上皮性肿瘤切片作阳性对照，用PBS液代替一抗作阴性对照。

　　1.3 结果判断

　　免疫组化染色，无棕黄色为(-);淡棕黄色为(+);棕黄色为();棕褐色为()。同样物镜下观察阳性细胞数，无阳性细胞数为(-);阳性细胞数≤10%为(+);阳性细胞数11%～50%为();阳性细胞数>50%为()。integrinα5β1主要定位于细胞质，极少数细胞核和细胞膜着色。

　　1.4 统计学处理

　　应用SAS6.12和SPSS10.0统计软件进行分析。半定量资料采用χ2检验;定量资料采用方差分析，χ2检验和秩和检验。

　　2 结果

　　2.1. integrinα5β1在不同卵巢上皮性肿瘤组织中的表达(见表1)表1 integrinα5β1在不同卵巢上皮性肿瘤组织中的表达(略)

　　integrinα5β1蛋白在卵巢良性、交界性肿瘤中的表达与卵巢恶性肿瘤中的表达差异均有显著性(P<0.01)。恶性肿瘤表达见图1～图3。

　　2.2. integrinα5β1与恶性卵巢上皮性肿瘤临床生物学行为的关系(见表2)表2 integrinα5β1表达与恶性卵巢上皮性肿瘤临床生物学行为的关系(略)

　　54例恶性卵巢上皮性肿瘤中，integrinα5β1的表达在各年龄段间没有显著差异(P>0.05);在不同肿瘤分级间的表达有显著差异，肿瘤分级越高，integrinα5β1表达越低，肿瘤分级越低，表达越高。进一步两两比较发现，Ⅰ级与Ⅲ级、Ⅱ级与Ⅲ间的表达有显著差异(P<0.01)，Ⅰ级与Ⅱ级间的表达差异无显著性。integrinα5β1在淋巴结转移组的阳性表达率较低为25.00%，未转移组的阳性表达率较高为61.11%，两组之间有显著差异(P<0.01)。

　　常瑞霞等.Integrinα5β1在卵巢上皮性肿瘤的表达及临床意义从表3可以看出，integrinα5β1在卵巢上皮性肿瘤表达的阳性细胞面积，按良性、交界性、恶性呈递减趋势，而灰度则相反，呈递增趋势。良性与恶性肿瘤Ⅲ级的阳性细胞面积相比较有显著差异(P<0.05);良性、交界性与恶性中Ⅲ级的灰度值相比较有显著差异(P<0.05);其它各组相比较无差异。

　　3 讨论

　　3.1. integrinα5β1与卵巢上皮性肿瘤的关系

　　整合素(Integrins)家族是黏附分子(CAMs)中的一个主要组成部分，由α和β两个亚基组成跨膜异二聚体，介导细胞与细胞外基质(extracellular matrix,ECM)、细胞与细胞之间及细胞与内皮细胞之间的黏附反应[2，3]。整合素蛋白α5亚基与β1亚基组成异二聚体，是基质的重要成分纤维连接蛋白(Fn)的受体(FnR)。integrinα5β1亚基在肿瘤组织表达水平减弱、缺失或在细胞表面的重新分布与癌细胞黏附细胞外基质能力的改变密切相关。Giancotti等[4]的研究结果显示integrinα5β1高表达，细胞之间黏附力增强，移动性减弱，侵袭性及转移能力下降。Zutter[5]等研究显示integrinα5β1的表达与肿瘤分化有关。本研究发现： integrinα5β1在卵巢良性、交界性和恶性上皮性肿瘤中的阳性表达呈递减趋势，组间均有显著差异(P<0.05)，提示integrinα5β1表达水平的改变，可能与肿瘤的发展和浸润有关，低表达与肿瘤的进展及恶性程度高有关。本研究中integrinα5β1的表达与淋巴结转移之间呈负相关关系，有淋巴结转移组的阳性表达明显降低(P<0.01)。提示integrinα5β1的表达可能在卵巢癌的盆腔淋巴结转移中起着重要的作用。

　　本研究利用计算机图像分析技术对integrinα5β1在卵巢上皮性肿瘤的表达进行定量分析，结果发现：integrinα5β1在卵巢上皮性肿瘤的表达，按肿瘤分级呈递减趋势，肿瘤分级越高，阳性细胞的面积越小，表达越弱;而灰度值则相反，肿瘤分级越高，灰度值越大，颜色越浅，表达越弱。以上结果我们可以看出，integrinα5β1在卵巢上皮性肿瘤表达定量与半定量的结果一致，二者呈相反趋势。相关性检测得出，二者呈负相关。

　　实验结果表明，integrinα5β1与卵巢癌的发展、侵袭和转移关系密切。定性和定量检测结果一致，为卵巢上皮性肿瘤的自动化分析、淋巴结转移及预后评估提供了新的思路和方法。
